When a Waiver Should Really Be a Form

Sometimes a FitPro might want to collect intake information from a new client and while it seems like a waiver, the FitHive system is build to receive intake information using forms.

Unlike the Waivers module, the Forms module is dynamic and built to receive customizable responses from your leads, members and users.

 

 

A waiver is a piece of information that requires a signature and is not supposed to change. The signature will be stored along with a timestamp that you can safely store for your records. 

 

 

 

When creating your intake form, it's important to make the distinction between form types.   If this is a new lead in your hive, then select "Lead Intake" and the system will automatically add contact fields like "name" and "email." 

 

 

If this form is meant for your members or for users already in your system then select "User" and, that way, the form will automatically be attached to the user who submits it.
